If you are a NEW PATIENT to our office, the forms below will need to be filled out when you arrive for your first appointment. Printing them, filling them out, and bringing them with you will allow us to attend to your dental needs more quickly than completing them when you arrive. If you prefer to fill them out at our office, please plan to arrive about 15 minutes early for your scheduled appointment time.
